The 12 numerator stays the same across every set and parallel, but the denominator (the X) changes: a 12/99 is the 12th of 99, a 12/25 is the 12th of 25, and so on. A card numbered 12/X is the 12th copy out of its parallel's print run, where X varies by set and parallel. A 12/99 is the 12th of 99, a 12/25 is the 12th of 25, and so on. The /X notation is collector shorthand for "of any print run."
